If you yourself have consent documents from previous operations, (medical) examinations, tests or research projects in which you have participated, we would be very happy if you could make them available to us.

We assure that no personal data that could be included on the consent forms will be published!

(Information: Currently we only work with documents that are used in Germany (in German).)
